This form is used by Program Directors and/or Students to review a remediation/dismissal/withdrawal form and either accept the submission or return it for revision. Once the submission has been accepted by the student, the relevant people will be informed and a copy will be placed in the student's record. It is also used by staff to close out remediation plans.
