About this process
NCAA rules require the University to collect information about outside scholarships given to student athletes. An outside scholarship is any scholarship received from an organization outside of the University of Minnesota Twin Cities (e.g. a local community organization, church, foundation, club, etc.).
The awarding agency is required to verify the recipient and provide details about the scholarship. This form is to only be completed by an official of the awarding agency.
This information is being requested in order to determine a student athlete's scholarship eligibility at the University of Minnesota Twin CitiesĀ and will be viewable by University staff with a business need. Declining to submit the information will prevent the University from finalizing the student athlete's eligibility. You are not legally required to submit the information and/or documentation. However, refusal to do so will result in an incomplete student athlete scholarship application, and therefore, they may not receive the scholarship they have been awarded.
Instructions
Complete all information as requested. This form should take you less than 10 minutes to complete.
The system will time out after 15 minutes of inactivity (not progressing to the next page of the form) and data may be lost.
